Privacy Policy
Who we are
Homeopathic Family Practice is operated by Pure, Simple and Amazing Inc. Our clinic is at 600 Sherbourne Street, Unit 604, Toronto, Ontario M4X 1W4, and we also see patients online from many parts of the world. This policy explains how we collect, use, share and protect personal information through our clinic, our website (homeopathicfamilypractice.com) and our online store.
Your privacy is part of your care. Personal health information is protected under Ontario's Personal Health Information Protection Act (PHIPA). Other personal information, such as store orders, is protected under Canada's Personal Information Protection and Electronic Documents Act (PIPEDA).
What we collect
- When you contact us: your name, email address, phone number and anything you choose to tell us in your message.
- When you book an appointment: your name, contact details, date of birth and appointment details. Online bookings are made through Jane, our booking and clinical records system.
- When you are a patient: the health history and information you share with your practitioner, notes from your visits, your care plan, assessment results and invoices. This is your clinical record.
- When you buy from our online store: your name, email address, phone number, billing and delivery address, and what you ordered. Your card number is entered directly into Square's secure payment form. It never passes through our website, and we never see or store it.
- When you visit our website: basic technical information such as the pages you view, your browser and device type, and your approximate location (city or region). This is collected through cookies and similar tools, including Google Analytics and Jetpack statistics.
- When you sign up for our newsletter or HFP Insider: your email address and name.
How we use it
- To provide your care, keep accurate clinical records, and communicate with you about appointments and your care plan.
- To process and deliver store orders, send order confirmations and receipts, and help with any issue with your order.
- To answer your questions and requests.
- To send newsletters and updates, only with your consent. Every email includes a way to unsubscribe.
- To understand how our website is used so we can improve it.
- To meet our legal, tax and professional record-keeping obligations.
We do not sell, rent or trade your personal information.
Your consent
We ask for your consent before collecting, using or sharing your personal health information, except where the law allows or requires otherwise. When you place an order, you consent to us using your information to complete it. You may withdraw your consent at any time, subject to legal and professional record-keeping requirements. We will explain what withdrawing may mean for your care or your order.
Who we share it with
We share personal information only with trusted service providers who help us run the clinic and the store, and only as much as they need to do their work:
- Jane (online booking and clinical records)
- Square (card payments)
- Bluehost (website hosting)
- Google Workspace (clinic email)
- Google Analytics and Automattic/Jetpack (website statistics, security and backups)
- OptinMonster and Substack (newsletter sign-up and delivery)
- Delivery couriers, for store orders delivered to you
These providers are required to protect your information and to use it only for the services they provide to us. We may also disclose information where the law requires it, or to another health care provider when you ask us to.
Where your information is stored
Some of our service providers store information outside Canada, including in the United States. Information stored outside Canada is protected by those providers' security measures, but it may be accessible to courts, law enforcement and national security authorities in that country. Your clinical records and online bookings, held in Jane, are stored in Canada.
How long we keep it
- Clinical records are kept for 20 years after your last visit. For a child, records are kept until 20 years after the child's 18th birthday. This meets or exceeds the record-keeping requirements of our practitioners' regulatory colleges.
- Store and financial records are kept for at least six years, as the Canada Revenue Agency requires.
- Newsletter details are kept until you unsubscribe.
When information is no longer needed, it is securely deleted or destroyed.
How we protect it
We use physical, administrative and technical safeguards. These include secure, encrypted connections on our website, password-protected systems with limited staff access, confidentiality commitments from everyone on our team, and secure payment processing through Square.
Your rights
You may ask to see the personal information we hold about you, and to have it corrected if it is inaccurate. You may also ask how it has been used or shared. We will respond within 30 days, as the law requires. In some limited cases the law allows or requires us to refuse access, and we will explain why.
